An exciting opportunity has arisen to join a dedicated and enthusiastic team that supports individuals preparing for discharge from hospital.Our team is committed to delivering high standards of care within a positive and collaborative working environment. We work closely with individuals to understand what matters most to them, carrying out Mental Capacity Assessments, Continuing Health Care Assessments, and leading Best Interest Meetings to ensure safe and timely hospital discharges.
We collaborate with a wide range of professionals, including colleagues from the health board, therapy services, and external providers. Ideally you will have;
Applicants will need to be social work qualified and professionally registered as a social worker with Social Care Wales.
You will need a knowledge and working practice of the Social Services and Wellbeing (Wales) Act 2014, to include outcome focussed assessment and care planning, knowledge and working practice of Continuing Health Care, the Mental Capacity Act 2005 including Deprivation of Liberty Safeguards.
