Salary: £24,920 - £27,514 per annum
37 hours per week worked on a rota basis of any 5 days out of 7, between 7 am to 11 pm, for which enhancements will be paid. A rolling rota exists to ensure service delivery.
Community Wellbeing Team, Cimla Health & Social Care Centre & throughout NPT County Borough
The Community Wellbeing Team is a multidisciplinary integrated service that works closely with the Swansea Bay Health Board. The team provides a Reablement service as well as short and long term Care to residents within NPT Borough.
You must be able to work as an effective team member of a multi-disciplinary, multi-agency team in co-ordinating the above and take the lead role for co-ordinating and liaising with professionals, individuals, and Community Wellbeing Team Support Workers to ensure a seamless transition between all service providers. You must also ensure the delivery of a high quality service in line with Registration and Inspections Social Care (Wales) Act 2016, statutory duties, policies, and procedures.
To undertake this challenging role, you must have experience of working with individuals in need of care or support in a health and social care setting; have an understanding of supporting people living with dementia; be registered or working towards your registration with Registration and Inspection of Social Care (Wales) Act 2016; and have experience of developing quality assurance systems, and performance frameworks.
You must have QCF qualification Level 3 in Care or be willing to work towards the qualification within a specified period of time. It is essential that you can travel extensively within the NPT authority therefore a full valid driving licence and access to a vehicle insured for Class 1 business use is essential.
You must be registered or work towards their registration with Registration and Inspection of Social Care (Wales) Act 2016.
